Main Employee Performance Review Forms

The performance review form has 3 different versions, depending on whether you manage other people or not.
It is important that you access and use the right version - there is a brief explanation with each form below.

Employees who do not manage other people

This version of the form (below) should be used with all employees who do not manage other employees, i.e. they are not reviewers of others.

Employee's who do not Manage others








Employees who are at or below grade 10 and who manage other people (or are Professors who manage other people but are not senior or executive managers)

This version of the form (below) should be used with all employees who either:
1) Manage other employees and are at or below pay grade 10
2) Are Professors and manage other employees but who are not senior or executive managers.

Employees who are senior or executive managers

This version of the form (below) should be used with senior and executive managers.
